This is a Contract Agreement with Eyehunter Guide Service and the Hunter/Hunters named above for a Self Guided hunt. I agree to Eyehunter Guide Service Rules and Regulations as well as the Ohio Division of Wildlife Laws and Regulations pertaining to Ohio. We understand that Self Guided Hunt fees are Non-Refundable and to be paid in full in advance within 6 days of our reservations, all terms are binding, and that we have read and understand this contract required of Eyehunter Guide Service. We also understand that we must submit all fees in Certified Check, Money Order or Credit card with paypal with our Contract to Hunt. This is free range Self Guided hunting and we understand there are no guarantees to our hunting success expressed or implied in any manner. We understand that this is a Self Guided hunt and that we are receiving no Guide assistance on our hunt that we are contracting. I supply my own stands and any hunting accessories I may need and that I have read the web site page and agree to what is provided and not provided, requiring no further assistance from Eyehunter Guide Service in the performance of this hunt. I understand that if I violate the Rules and Regulations of Eyehunter Guide Service or Ohio Game Laws and Regulations that I will forfeit all moneys received by Eyehunter Guide Service and subject to the Laws and Penalties of Ohio. We understand that we must meet all Ohio hunting license and permit requirements. We also understand that we must submit with this contract and fees due a Waiver of liability and terms of service form as outlined by Eyehunter Guide Service. Ohio serves as Jurisdictional State.
